Monday, 9:00am-5:00pm, Tuesday, 9:00am-5:00pm, Wednesday, 9:00am-5:00pm, Thursday, 9:00am-5:00pm, and Friday 9:00am-4:00pm.
Appointments
An appointment is required to see the doctor. Our available appointment times range from 10:30am to 4:45pm. Please make sure you always bring your most current copy of your insurance card and your photo ID to your appointment. Federal law now requires that we have a current copy on file at all times. We make every effort to be on time but unfortunately delays and emergencies do occur. If you arrive 15 minutes late for your appointment without calling, we reserve the right to reschedule your appointment.
Telephone Calls
Our phones are answered daily from 9:00am to 5:00pm, Monday through Thursday; and 9:00am to 4:00pm of Fridays. Each phone call is prioritized by urgency. We make a valid effort to answer all messages left promptly.
After Hours Calls
If you contact our office after business hours you will reach our answer machine. This will have further instructions for contacting the answering service in the event of an emergency situation.
Medication Refill Policy
It is faster to have you pharmacy contact us electronically to refill your medications. If you do not feel comfortable with this, you can contact our office and speak with a receptionist. Please allow at least one business day for us to refill your medication. No refill will be given on weekends. We also recommend that refills are requested prior to running out of medication to accommodate delays that may arise due to insurance authorization etc. Please note that certain controlled substances cannot be e-scribed and a paper prescription will be issued and that requests for narcotics will only be addressed in an office appointment.
Confidentiality
We firmly believe in patient rights. We will not release your medical records without your signed consent. If you would like anyone besides yourself to have access to your record, please specify them as your personal representative.
Medical Records
Current medical records will be available for pick up, fax or mail within 7 business days. Our fees for duplicating your records are as follows: Pages 1-20 $1.00/page; pages 21-50, $0.50/page; pages 51 and over, $0.20/page. If you require a copy of records prior to 2009, there is an additional retrieval fee of $10.00 as they must be retrieved from an off-site storage facility.
Referrals and Authorizations
Some insurance companies require referrals and/or authorizations to see a physician. It is the patient’s responsibility to provide our office with a referral. If you do not you will be responsible for all incurred charges.
Many diagnostic tests and medications require insurance authorization. We are happy to obtain an authorization for you, but they can take up to two weeks to obtain from your insurance company. Please be patient as we try to assist you in this process.
